What Does the Indiana Driver Safety Program Course Survey Ask?
Near the end of the Indiana Driver Safety Program course, you will complete a short 5-question survey as part of the normal course flow. The survey is required before the course ends. Your certificate is issued immediately once the course — including the survey — is complete.
The 5 Survey Questions
The survey questions are set by the Indiana BMV and are the same for all students:
Question 1: What suggestions do you have for improving a student's experience? (Open text — write anything you would like or enter N/A if you have no suggestions) Question 2: How would you rate the difficulty of this Driver Safety Program course on a scale of 1 to 10? (1 = not difficult, 10 = very difficult) Question 3: How would you rate your overall experience completing this Driver Safety Program course on a scale of 1 to 10? (1 = very dissatisfied, 10 = very satisfied) Question 4: After completing the Driver Safety Program course, what is something you learned that you will keep in mind when driving? (Open text — share one thing from the course) Question 5: How likely are you to recommend this course to family or friends? (1–5 star scale, where 1 star = least likely and 5 stars = very likely)
Why Is the Survey Required?
The Indiana BMV requires all Driver Safety Program providers to collect feedback from students as part of the program. Your responses help the BMV monitor course quality across all providers. Driving Logic submits survey results to the BMV on a monthly basis.
The survey does not affect your certificate. There are no right or wrong answers. You simply need to complete it.

The survey takes about 2 minutes to complete. All five questions must be answered before you can finish the course.
