Why This Matters
Courts and the Tennessee DSHS use the information on your certificate to match it to your driving record. If your name, driver license number, or date of birth does not match what the court or DSHS has on file, your submission may be delayed or rejected.
Before submitting your certificate, verify that the information on it matches your driver license exactly. If anything is incorrect, contact us before submitting — we can correct the certificate before it goes to the court or DSHS.
What Is Not on the Certificate
Your certificate does not include:
Your quiz or exam scores
The number of attempts it took to pass
Your home address

Frequently Asked Questions
What if my name on the certificate does not match my driver license? Contact us before submitting your certificate. We can correct the name on your certificate. Submitting a certificate with a mismatched name to a court or the DSHS may cause your submission to be rejected.
What if I used a nickname instead of my legal name when I enrolled? Courts and the DSHS require your legal name. If the name on your certificate does not match your driver license, let us know and we can update it.
My certificate shows the wrong driver license number. What should I do? Contact us right away. We will correct the driver license number before you submit. Do not submit a certificate with an incorrect driver license number.
Does my certificate look official enough for court? Yes. Your certificate is generated in accordance with Tennessee DSHS requirements and identifies Driving Logic as an approved provider. If a court questions its validity, let us know and we can provide additional documentation.
