Step 1 — Check your email
Driving Logic emails you Form 4444 immediately after you complete the course. Check your inbox for an email from Driving Logic with your certificate as a PDF attachment. If you do not see it, check your spam or junk folder.
Step 2 — Print your Form 4444
Print the Form 4444 PDF on standard 8.5 x 11 paper.
Step 3 — Follow the submission steps for your approval type Court-order customers: Take your printed Form 4444 to the court that approved your participation. A court clerk signs and dates it. You then mail the signed Form 4444 to the Missouri Department of Revenue. The DOR mailing address is on the bottom of Form 4444.
