Step 1 — Check your email
Look for an email from Driving Logic with Form 4444 attached as a PDF. It is sent immediately after you complete all course requirements. Check your spam folder if you do not see it in your inbox.
Step 2 — Print Form 4444
Print the PDF on standard 8.5 x 11 paper. A digital version on your phone is not sufficient — your court requires a physical copy.
Step 3 — Take it to your court
Bring the printed Form 4444 to the court that approved your DIP participation. Ask the clerk to sign and date the certificate.
Step 4 — Mail Form 4444 to the Missouri DOR
